GENERAL FUNCTION

The Community Health Coordinator will develop partnerships with stakeholders in the priority population to advance mutual YMCA and Morgan County Health Department community health equity goals and successfully achieve program and grant outcomes.  

The Community Health Coordinator will assist with the recruitment and enrollment of participants into community health programs; including but not limited to the YMCA’s Diabetes Prevention Program, and Blood Pressure Self-Monitoring and helping county residents achieve their physical health and well-being goals. Coordinator will guide and grow group fitness efforts in the county. 

KNOW HOW

A bachelor’s degree in public health, social work, or related field preferred. Must have an existing Community Health Worker Certification or 1-3 years’ experience leading/facilitating classes and working with special populations, or willing to obtain Community Health Worker Certification within first 6 months of hire. Experience in basic program management and supervision of volunteers/staff preferred. Knowledge of basic health and wellness principles and exercises, as well as healthy living behaviors is preferred. Must have exceptional communication and organizational skills in order to support participants and maintain required documentation for program. Knowledge and experience with health behavior and challenges associated with making lifestyle changes preferred. Verbally provide clear concise directions and visually inspect classroom for safety. Ability to maintain confidentiality of screening participants. Must be willing to complete YMCA provided trainings and certifications in HIPAA, CPR/ AED, First Aid, Safety & Risk Management, Child Abuse Prevention and New Employee Orientation with the ability to obtain within 60 days of employment. 

PRINCIPLE RESPONSIBILITIES

  • Strategize and implement programs to reduce barriers to access and improved overall health to the population served.
  • Identify and address individual and community needs around health education and access to resources. 
  • Provides community resource referrals, participant advocacy, support, outreach, and follow-up.
  • Provide instruction to class participants in effective and meaningful ways, when teaching.
  • Build relationships and collaborate with community partners.
  • Develop and maintain relationships that promote the building of small communities.
  • Create a motivating environment that is friendly and non-competitive.
  • Be available to screen participants both before and after outreach events, to answer questions, and follow-up on any questions you cannot answer during event time.
  • Maintain Continuing Education Credits and other trainings to keep all certifications current.                            
  • Conduct self in accordance with the YMCA Core Values; Caring, Honesty, Respect, Responsibility.
  • Enter data into appropriate data system immediately after class, and securely access/utilize/store data in accordance with privacy practices.
  • Available to lead/ attend programs or meetings in the evenings and weekends at multiple sites/locations. 
  • Assist with other projects or activities as needed.

PHYSICAL AND MENTAL REQUIREMENTS

  • Ability to stand and perform repetitive movements for extended periods of time.
  • Ability to bend at the waist and knees.
  • Ability to lift 40 pounds and transport equipment.
  • Ability to visually inspect equipment and complete preventative maintenance checklist as required.
  • Ability to communicate effectively with participants, staff, volunteers and community partners.
  • Ability to demonstrate and lead activities.
  • Ability to solve problems/address participant concerns and use sound judgment even when distracted by noise and activity.
  • Ability to observe and promote safety and security procedures.
  • Ability to physically and mentally respond to emergencies and administer CPR and First Aid effectively.

  • Be informed and up to date on the latest developments in your field.
  • Acquaint members with other YMCA programs and benefits.
  • Develop relationships that promote the building of small communities.
  • Recommend participation in a variety of YMCA programs.
  • Be able to articulate the role and importance of volunteers.
  • Be informed about volunteer opportunities.
  • Provide an environment in which valuing diversity is encouraged and the 40 Developmental Assets cans be built and emphasized.

EFFECT ON END RESULT

  • Evidence of maintenance and/or increased attendance of assigned classes.
  • Individuals have learned new skills and have had their life enriched through the class experience.
  • Creation of a safe and nurturing environment for program participants, members, volunteers and staff.
